脱细胞生物羊膜对甲床手术创面恢复的作用

Effects of Human Acellular Amniotic Membrane on Postsurgical Recovery of Nail Beds

  • 摘要: 目的 明确脱细胞生物羊膜应用于甲床二期愈合的甲外科手术中,是否有促进创面愈合的作用。方法 将行甲床手术后创面二期愈合的患者随机分配到治疗组( n=19)和对照组( n=19),甲床手术后第1天换药时,治疗组使用羊膜后覆盖凡士林纱布;对照组仅覆盖凡士林纱布。记录换药后1 h、4 h、8 h、12 h、24 h、36 h、48 h、72 h患者疼痛评分;记录创面愈合时间、甲生长时间、患者和医生对外观的满意度。结果 两组患者基线一致,从换药后4 h开始至换药后48 h,两组疼痛评分差异有统计学意义( P<0.05)。换药后24 h治疗组全部患者疼痛消失。创面愈合时间:对照组平均需要(17.68±2.86) d,治疗组则需要(12.37±2.48) d,两者差异有统计学意义( P<0.05)。甲床手术后第1天到甲完全生长所需时间:对照组平均需要(15.63±2.00)周,治疗组则需要(13.95±1.72)周,两者差异有统计学意义( P=0.016)。甲完全生长后,甲单位外观满意度评分:对照组(7.58±1.18)分,治疗组(7.61±1.38)分,两组间差异无统计学意义( P=0.919)。结论 脱细胞生物羊膜可以促进甲床手术创面愈合。

     

    Abstract: Objective To determine the effects of human acellular amniotic membrane(HAAM) on postsurgical recovery of nail beds. Methods Patients undergoing nail surgery were randomly divided into two groups: 19 in the experimental and 19 in the control groups. Patients in the experimental group were given HAAM before the wounds were covered with vaseline gauze the next day after surgery; whereas those in the control group received vaseline gauze dressing only. Ratings of pain were recorded when 1, 4, 8, 12, 24, 36, 48 and 72 h after the dressing was changed. The time when the wounds healed and the nail grew thoroughly were observed. Results The characteristics of patients in the two groups were similar. The patients in the experimental group reported significantly less pain at 4 h and 48 h than those in the control group ( P<0.05).After dress changing 24 h, pain disappear in the experimental group. The wounds of the patients in the experimental group also healed quicker 〔(12.37±2.48) d〕than those in the control group 〔(17.68±2.86) d, P<0.05〕. It took (13.95±1.72) weeks for the patients in the experimental group to have their nails grown completely, compared with (15.63±2.00) weeks for those in the control group ( P=0.016). The experimental group had a similar degree of satisfaction (7.61±1.38) towards the cosmetic appearance of their nails compare the control group(7.58±1.18, P=0.919).Conclusion HAAM can promote healing of surgical wounds on nail beds.
