布鲁赫膜在近视发生与发展中的机制研究

Mechanism of Bruch’s Membrane in the Occurrence and Development of Myopia

  • 摘要: 近视是一个正视化失代偿后眼球壁重构、眼轴不断增长的过程,但在这个过程中,眼底结构特征性变化之间的因果关系尚不明确。脉络膜位于视网膜和巩膜之间,在与近视相关的信息传递过程中起着承上启下的作用,其在近视眼中的作用是近几年研究的热点。动物实验发现,形觉剥夺引起的脉络膜厚度变化可能与其血管灌注情况有关,但近视化过程中引起脉络膜的血管灌注发生改变的启动机制仍需探讨。布鲁赫膜(Bruch’s membrane)是一种位于脉络膜前的具有良好舒缩能力的弹性膜组织。在近视化过程中,布鲁赫膜的合成或生物力学发生区域性的变化,可能是最早引起脉络膜厚度与血流变化的结构基础。本文以脉络膜厚度作为切入点做一综述,着重探讨布鲁赫膜在近视发生与发展过程中的作用机制,进一步深化对脉络膜厚度变化机制的理解,为开发新的近视治疗靶点提供理论依据。

     

    Abstract: Myopia is a process of ocular wall remodeling along with axial elongation after emmetropia decompensation, but the causal relationship among the changes taking place in ocular fundus structures during this process is not clear. The choroid, which lies between the retina and the sclera, plays an important role in the transmission of information related to myopia. The role of choroid in myopia is a hot research topic at present. Findings from animal experiments showed that form deprivation-induced changes in choroidal thickness may be related to the vascular perfusion, but the triggering mechanism of choroidal perfusion changes during the process of myopia still needs to to be further explored. Bruch’s membrane is an elastic membrane located in the front of the choroid with good contractile properties. In the process of myopia, regional changes of the synthesis or biomechanics of Bruch’s membrane may have formed the earliest structural basis of changes in choroidal thickness and blood flow. Taking choroidal thickness as a starting point, this paper focuses on the role and mechanism of Bruch’s membrane in the occurrence and development of myopia, which may further deepen our understanding of the mechanism of changes in choroidal thickness, and provide a theoretical basis for the development of new therapeutic targets for myopia.
